The date (06.22.16) adds up to 22, the digits of his total at the cash register (4.99) add up to 22, and the speeds multiplied together equal 22. If you were counting, you’d notice that Ellias meets 11 people. Therefore, to get to 22, you’d have to multiply by two which means that two people killed Ellias.
You probably guessed that the gaunt looking man had something to do with it. He was the lookout. What you probably didn't realize is that the driver in the beginning was Isabella’s father. The hint was that he banned his daughter from driving until she was 18. He is terrified of driving in the snow and he blamed Ellias for killing his daughter.
His daughter died at the age of 22 which is why he is fixated on the number 22. He hired a private investigator, the gaunt man, to track down Ellias and determine his habits. The gaunt man develops ill will towards Ellias and agrees to be lookout. They hatched a plan to kill him on the day of 22s.
When he hears Ellias yell, that’s confirmation that his plan is working perfectly. He drove slow enough to let Ellias pass him and get to the gas station before he did. There, the father cut his brake lines. This is illuded to in the line about lunch breaks.
The gaunt man was prepared to interfere if Ellias got his food before the brake lines were cut. However, the blue haired woman slowed him down enough that the gaunt man didn't have to interfere.
